Homocysteine is an intermediate compound that is normally metabolized via three different pathways. A small amount of homocysteine can be cleared via the kidneys. Several vitamins/cofactors (such as B6, B12, folate, and betaine) and minerals (such as Zn and Mo) are needed for the proper functioning of these pathways:* 

The body manages homocysteine levels in different ways:

1) Folate-dependent remethylation Homocysteine receives a methyl group from 5-methyltetrahydrofolate (5-methyl THF) to form methionine—a reaction catalyzed by the enzyme methionine synthase (MS)—and requires the vitamins B2, B12, and 5-methyl THF. Methionine is then converted to S-adenosylmethionine (SAM), the main methyl donor in the cell.*

2) Folate-independent remethylation Homocysteine receives a methyl group from betaine and forms methionine, catalyzed by the enzyme betaine-homocysteine methyltransferase (BHMT). Methionine is then converted to SAM, the main methyl donor in the cell.*

3) Transsulfuration Homocysteine is irreversibly metabolized to form cysteine, facilitated by the enzymes cystathionine—cynthase (CBS) and cystathionine-lyase (CTH). This pathway requires vitamin B6.*

4) Renal clearance The nutrient NAC has been shown to support renal clearance of homocysteine.*

Remethylation of homocysteine to methionine is essential for the synthesis of SAM. Elevated homocysteine contributes to a build-up of S-adenosylhomocysteine (SAH), a by-product of all SAM-mediated methyltransferase reactions. SAH is a potent competitive inhibitor of almost all methylation reactions dependent on SAM as the methyl donor. Thus, reducing homocysteine may reduce the buildup of SAH in the cells and may help support healthy cellular methylation capacity.*
